The Fairfield County Health Department is here for you and your family.


We have a Community Health Worker (CHW) who can help you with things like:

 

  • • Getting food from local pantries
  • • Filling out forms and applications
  • • Finding a doctor, dentist, eye doctor, or mental health care and more.

Our CHW will talk with you about what you need and what’s hard for you right now.

Then, they will help you find services in Fairfield County that can help.

This help is free for anyone who lives in Fairfield County.

For the Community Health worker referral, when you select the reading option it reads out all of the underscores out this makes it to where you can't listen to what the form is asking for instead of only hearing a repeat of "underscore" for extended periods of time.
