Clinic locations
- FCHD Main Office, 1550 Sheridan Drive, Suite 100, Lancaster, OH 43130. Clinics held on Tuesdays.
- Pickerington Office, 11050 Tussing Road, Pickerington, OH 43147. 3rd Wednesday of the month.
- Off-site clinics for flu and COVID-19 can be held at workplaces, churches, schools, senior centers, and other locations. Call 740-652-2802 for more information.
