How to report

Reporting requirements

Class A, report immediately by phone

Diseases of major public health concern because of severity or potential for epidemic spread, report immediately by telephone upon recognition of a case, suspected case, or positive laboratory result.

  • Anthrax

  • Free-living amoeba infection
  • Influenza A, novel virus infection
  • Measles
  • Meningococcal disease
  • Middle East Respiratory Syndrome (MERS)
  • Plague
  • Rabies, human
  • Rubella (not congenital)
  • Severe acute respiratory syndrome (SARS)
  • Smallpox
  • Tularemia, inhalation
  • Viral hemorrhagic fever (Ebola, Lassa, Marburg, Crimean-Congo)

Also report any unexpected pattern of cases, deaths, or increased incidence of any disease of major public health concern that may indicate a newly recognized agent, outbreak, epidemic, public health hazard, or act of bioterrorism.

Class B, report by end of next business day

Diseases of public health concern needing timely response, report by the end of the next business day after a case, suspected case, or positive laboratory result is known.

Class C, report outbreaks & unusual incidents

Report an outbreak, unusual incident, or epidemic of other diseases (e.g., histoplasmosis, pediculosis, scabies, staphylococcal infections) by the end of the next business day. Outbreak types include:
