If you are not the biological parent of a child coming for vaccines, we MUST have a written note from the parents or guardianship papers on file. If we do not have this information, you will be asked to reschedule when you will be able to provide documentation. This is a legal requirement.

PUBLIC HEALTH NURSING
The Public Health Nursing division addresses chronic, acute, and communicable diseases through community planning, health education, and clinic services designed to meet community needs and support the mission to prevent disease, protect the environment, and promote healthy lifestyles. All public health nursing services are provided under the direction of Gale Neville, MSN, RN, director of nursing with guidance from Steven Kapetansky, MD, MPH, Medical Director.
